Our goal is to help you make your dental visits more affordable so that you can get the quality dental care you want and need. Our very own in-house plan will provide you with tremendous savings.

This reduced fee dental plan allows individuals or families to receive all the available dental services at Sea Lotus Dentistry for a percentage discount off the office regular fees.

Savings

 

Plan 1- Membership includes the following services at NO CHARGE!!  $

  • 2 routine exams (Any time throughout the contract year)  

  • 2 limited problem focus exam, x-ray PA included  

  • 2 routine cleanings (Any time throughout the contract year)  

  • X-rays (4Bite-wings, 2PA’s – throughout the contract year) 

  • 2 Fluoride treatment for children (under the age of 19) 

ADULTS = $269                CHILDREN = $189 (under 13yrs not include 13yrs)

Plan 2 – Membership includes the following services at NO CHARGE!!  $ 

  • 2 routine exams (Anytime throughout the contract year)

  • 2 limited problem focus exam, x-ray PA included

  • 4 periodontal maintenance (every 3 months)

  • X-rays (4BW’s, 2PA’s – throughout the contract year)

Adults = $ 469 

 

In-House Dental Plan Membership

Once the annual fee is paid and received for the membership program, you will be entitled to discounted-fee dental services for 12 months. Members, spouses, and dependent children under the age 19 or full time students up to 23 years of age are eligible. Children may be members only as dependents of family members.

  • All dental services are offered at 20% fee reduction off our normal fee. Discount does not apply to “special offers’ the office runs on discounts pricing. 

  • Co-payment must be paid at the time of service.

  • Any services not paid at the time of service will be billed at usual and customary Fees.

  • Membership valid for one year from date of sign-up

Patient agrees that Sea Lotus Dentistry in-house plan fees stated must be paid at the time of services are rendered. Any service not paid at the time of service will be billed at usual and customary fees. Plan fees are valid only when paid at the time of enrollment. All family members must reside in the same house hold. (This is not an insurance program, Sea Lotus Dentistry is not a licensed insurer, health maintenance organization, or other underwriter of health service. This plan con not be used in conjunction with any dental insurance or other discounts or advertisement. The membership plan is only valid with Sea Lotus Dentistry.)The member has the right to opt out of the plan for a refund within 30 days from enrollment as long as treatment has not been started. If any treatment has been performed or if 30 days from enrollment have lapsed, NO refund will be given. Cancelling the membership plan will result in a $50 dollars transaction fee. The member will be responsible for paying the remaining balance regardless of service rendered.
